Required Documents

Application forms required for entry into EPOK should be completed and submitted to the International Exchange Office of the student’s home university. Further information regarding EPOK and Okayama University is also available at the student’s home university. The following list outlines the documentation requirements for the EPOK application:

  • EPOK application form
    * Application form should be typewritten.

  • Proposal for Independent Study
    * Proposal should be typewritten.

  • Two (2) recommendation letters written by faculty members/staff of the home institution (written in either Japanese or English)

  • Transcript of the applicant’s academic record from their home university
  • Certificate of Health

  • Application for Certificate of Eligibility

  • Certificate of Enrollment of the Applicant for JASSO Short-Term Student Exchange Promotion Program (Inbound) Scholarship

  • Four (4) passport size (4 cm. by 3 cm.) photos taken within three months, included with your application (name written on the back of each photo)
  • A copy of the photo page from your valid passport (name, date of birth, etc. must be visible)
  • TOEFL score record (for applicants from non-English speaking countries)
