Regarding the COVID-19 Vaccine Booster Shots(3rd Dose)

The students and faculties are able to take the 3rd dose that product of moderna. The vaccination is voluntary. We will not restrict activities such as education, reserch, using facility in Okayama university to all of you, whether or not you vaccinated.
For those who would like to get the 3rd dose, please make a reservation after you read the entire text below and make sure that you understand its contents.


・If you don't take your 1st and 2nd dose yet, you can NOT take vaccination here this time.
・Even though you took your 1st and 2nd dose at off-campus, you can take your 3rd dose here.

Please note that we outsource the registration of vaccination data for the government system with prevaccination questionnaire, throughout our COVID-19 vaccination project.
The personal information will never leak because we contract with outsourcing company for the confidential information, and we keep under surveillance about handling personal information by outsourcing company.



1.Vaccination Period
Large-scale venue:
-March 15th, 16th, 17th, 19th, 23rd, 24th, 25th, 28th, 29th, 2022
Small-scale venue:
-April 11th, 2022 - September, 2022(Excluding Saturdays, Sundays and National Holidays)

 

2. The persons this vaccination program applies to:
-18 years of age or older
-All students(including reserchar), all faculities(including part-time teacher), all staffs(including campus store's staff)

-At least 6 months after completing your initial COVID-19 vaccinations
-Already received the documents(including COVID-19 Vaccination Voucher) from the municipality office

You will receive the envelop from municipality office when you can take a booster shot(3rd dose). After that, please make a reservation through our reservation system.

NOTE!
You can NOT take vaccination if you don't bring the documents on the vaccination day. Please be sure to receive the vaccine documents from municipality office before you make a reservation.
Please check the homepage of municipality where you are a registered resident to find out when your vaccination voucher is due to arrive.

For those who will graduate or retire on March 2022:
You can access our reservation system until March 25th, but you can NOT make a reservation or change reservation after that day.

Large-scale venue:
-Gymnasium No.2 at Tsushima campus
Small-scale venue:
-(The venue from April will be announced at a later date.
)





5. Things to bring
Please make sure you bring the following three items

・Combines Vaccination Coupon and Pre-vaccination Screening Questuinnaire
・Certificate of Vaccination for COVID-19

・Confirmation of your current address(e.g. residence card, students ID, staff ID, driver's licence, passport)

The certificate will be sent out along with your Vaccination Coupon. On the day of your vaccination, a sticker showing the batch number of your 3rd dose will be attached to the 'Certificate of Vaccination' document, and it will serve as a record of your vaccination history.

6. Note
 ・Please wear clothes that make it easy to access your shoulders.
 ・Please wear a non-woven mask.

Reservation Period

For undergraduate students who is enrolling the medical related department and received the second dose during July 2021
-February 22nd(Tue.) 9:00 am -


For students and faculties who will graduate or leave Okayama University:
-February 24th(Thu.) 9:00 am - March 25th (Fri.)
-No reservations or changes can be made outside of this period.
-Vaccination can be at Large-scale venue only.


For all students and faculties excluding above, and family members of faculties:
-February 28th (Mon.) 9:00 am -
