先に実施した学生定期健康診断をやむをえず受診できなかった学生は、下記のとおり、追加の健康診断を実施しますので受診してください。なお、予備日を設けておりますが、可能な限り、実施日に受診していただきますよう、よろしくお願いいたします。

なお、今回の追加健康診断においては、保健管理センターホームページ

健康診断予約システムにて事前予約の上、予約した受付時間内にお越しください。

予約開始日：令和元年5月20日（月）
実施場所： 岡山大学保健管理センター(津島キャンパス）

※健康診断の項目
●身体計測(身長・体重)
●胸部Ｘ線
●検尿(たん白・潜血・糖)
●血圧測定
●内科診察
●視力検査(希望者)
●心電図検査（内科診察で指示があった方のみ）

※受診上の注意
①学生証を持参してください。

②尿検査は受診時に採尿してもらいますので、直前にはトイレに行かないようにしてください。

③服装の注意
　◎着脱しやすい服装で受診のこと。
　◎貴重品、イヤリング、ネックレス等は紛失のおそれがあるので持参しないこと。
　◎長髪の方は、髪を束ねるゴム等を持参のこと(胸部Ｘ線検査時に髪が胸部分にかからないように)。 　

※健康診断の結果について
◎追加健康診断の結果は、６月２8日以降に学内のパソコンから各自で見てください。(学務システム学生用webから)
是非結果を見てご自分の健康管理に役立ててください。

◎※マークのついている項目は再検査、精密検査等の必要な項目です。
早めに保健管理センターへおいでください。

◎健康診断の結果で尋ねたいこと、相談したいこと等ありましたら、保健管理センターへ気軽に来てください。

※健康診断書について
健康診断証明書は６月２8日以降、学内の証明書発行機にて発行できます。
★健康診断結果に※マークのある方は発行できません。

★「健康診断書」を希望される場合は保健管理センターへご相談ください。

★健康診断未受診者には発行できません。
また、指定の書式や病院指定のある場合等 発行できない場合もあります。
発行は年度内とします。



Students who missed the health check-up in April should be sure to receive another one as shown below.

Please make a reservation in advance by the health check-up reservation system (Japanese only) on the Health Service Center website
( http://www.okayama-u.ac.jp/user/hokekan/kenshinyoyakusisutemu.html ), and come within the time of booking.
The reservation starts from May 20 (Mon).
Student ID card is needed when you receive a health check-up.

RESULTS OF HEALTH CHECK-UP

You can browse the results of your health check-up in the Okayama University Official Home Page using PC on　
campu from June 28. Please check it for your health management. If you can’t check the results, please contact us.
＊indicates the need for re- or through examination. Please contact us as soon as possible.

HEALTH CERTIFICATES

The Health Service Center will issue a health certificate free of charge only to students who have received the
health check-up. It would therefore be quite beneficial for you to receive it.


NOTE　

・Student ID card is needed when you receive a health check-up.

・Women may wear a thin plain clothes (e.g. a T-shirt) for chest X-ray examination.

・If you do not receive a health check-up, it might obstruct future application procedures for a National Health
Insurance or scholarship.
