How to obtain various certificates

You will need the following items to apply for a certificate.
Applications must be made at a school office counter or by postal mail; applications by e-mail, fax, or telephone are not accepted.

There is no fee for issuing a certificate.
Submitted documents at the time of application will be used only for issuing a certificate. These documents are duly managed according to the law and are disposed of after the designated storage term.

■ Application process: At a School Office Counter

Your affiliated faculty or graduate school office is responsible for issuing a certificate of graduation.

Office hours:
Monday to Friday 8:30 am to 5:00 pm
NOTE: Closed on public holidays, year-end and New Years holidays (Dec. 29th to Jan.3rd), and other university holidays.

Ⅰ-1 To apply at a School Office Counter (Application by Proxy)

(Submission)
Photocopy of driver's license, passport, national health insurance card, residence card, a valid Okayama University student card, or any other official identification document.
2Any name changes after graduation
(Submission)
If your name changed after graduation or upon completion of your studies, submit a photocopy of a legal document as proof of your name change.
3Identification documents of the proxy
(Presentation)
Present driver's license, passport, health insurance card, residence card,a valid Okayama University student card, or any other official identification document. A legal document to confirm that you are the principal must also be submitted.
4Power of attorney
(Submission)
Submit a power of attorney (See sample PDF)
