ご挨拶

名誉教授　吉野　正

病理学は病気の成り立ちを研究する学問として発展し、現在のいろいろな学問体系のもととなっています。

私どもの教室是は、故濱崎名誉教授が掲げられた『人体病理、実験病理を両輪としてともに重視する』というものです。この立場は、故小川勝士名誉教授、赤木忠厚名誉教授によって受け継がれ今日に至っています。

この間、疾患概念の変遷と多様化、検索方法の進歩、治療手段の開発はめざましいものがあります。例えば、私どもが専門にしている悪性リンパ腫は、つい半世紀前には「円形肉腫」などと診断されることもありましたが、現在ではそのような診断名では不十分です。また、同じ白血球の腫瘍でも骨髄球系腫瘍との鑑別は治療方針に大きく影響するので、両者の鑑別は厳密にすることが求められています。「白血病みたいなもの」では到底合格点が得られないのです。

このように専門性が細分化し、知識量が膨大なものとなっている現在、ひとりの病理医が両分野に完全に通暁することは難しい状況となっています。幸い、本教室は現在まで多数の病理医、研究者を輩出しており、諸先輩、同僚、教室員のご協力を得ながら全体として教室是を貫いてゆきたいと念願しております。また、従来から学内、学外の諸教室、施設との共同研究を行っていますが、これを一層推進する態勢を整えてきています。

病理は基礎的研究の中だけにとどまりません。中核的総合病院で常勤病理医の果たす役割はその重要性がますます増しています。このように間口が広い立場は、やや散漫になる可能性もありますが、幅広い人材を擁する長所を最大限発揮することにより、臨床すなわち患者さんに還元できる成果を目指すという目標に向かって着実に努力してゆく所存です。

Introduction of the Department of Pathology (Oncology), Okayama University by Tadashi YOSHINO, MD, PhD, Emeritus Professor

My major is pathology, especially malignant lymphomas. We make diagnosis approximately 1300 of lymphomas per year which accounts probably for 10% of whole Japanese cases. I am asked to be a member of central diagnosis of almost all Japanese clinical research groups. Members of my department have reported a lot of papers on clinicopathology and pathogenesis of lymphomas, reviews, and textbooks, and have lots of chances of special lectures in or outside Japan. My research subjects are not only clinicopathology of lymphomas but adhesion molecules and apoptosis which are relaled to lymphomagenesis. For example, we found out characters of adhesion molecules in several lymphomas such as GI tract lymphomas and NK-cell lymphomas. We also focused on germinal centers which plays a key role in B-cell selection in peripheral tissues. I was chosen as an author of the fourth WHO classification published in 2008. Though my major is specific for hematopathology, "pathology" should cover very wide areas of medicine. Actually, my colleagues work for about 30 hospitals. As the progress of pathologic field is rapid and prominent, one pathologist cannot cover all fields of pathology. We have attempted to ask the pathologists related to our department to have their own major fields, that is more than ten organ or disease specific fields, to be authorities. My department have been making best efforts to get new members for pathologists, and probably all related hospitals can have more than one pathologists in future.
