Outpatient service procedures for first-time visitors to the Medical Outpatient Clinic

Counter No. 1.First-time visitors on the 1st floor, Medical Outpatient Clinic

Reception hours/ 8:30AM to 12:00PM
When you arrive before 8:30AM, please take a number at information counter on the 1st floor .
Closed/  Saturdays, Sundays, national holidays, and the year-end and New Year holidays

  • Please make sure to bring a referral letter, a public health insurance card of Japan , together with a photo ID card (passport, residence card or or alien registration card), etc.
  • If you are under treatment at another hospital,you need a referral letter.
  • We may not be able to accept a patient without a referral letter in some cases. 
  • Do you have a "primary care clinic or doctor"?
  • A patient without a referral letter must pay “Elective Treatment Fee” of 13,200 yen(inc.tax) in addition to the consultation fee.

Fill out the registration form, and show a referral letter, a public health insurance card of Japan , together with a photo ID card (passport, residence card or or alien registration card), etc. at Counter No. 1.
 
 
After registration, you will receive a patient ID card and an information sheet.Please show them at each department reception.
 Consultation

The scheduled consultation time may change depending on the situation.
We appreciate your understanding.

 
 Counter No.3 on the 1st floor, Medical Outpatient clinic

After your consultation, please return the “Today’s Information sheet” to the Counter No. 3 (Return Slot) on the 1st floor, Medical Outpatient clinic. Upon returning the slip, you will receive your payment waiting number.
If you came by car, please present your parking ticket at the Information Counter on the 1st floor, Medical Outpatient clinic.
(After 3:30 p.m., please present your parking ticket at Counter No. 3.)

 
Billing

When the payment calculation is completed, your waiting number will appear on the electronic display board.

Payment

When you insert your patient ID card into the automatic payment machine, today's charges will be displayed.
After completing the payment, please take your receipt (including your next appointment slip and medication voucher) along with your patient ID card.
For credit card payments, we accept the following: JCB , AMEX , VISA ,Master Card , Diners Club and Discover. 

 
Prescription of Medications

At our hospital, medications are generally provided through external prescriptions. We appreciate your cooperation. 

In-Hospital Prescription

Please bring your medication voucher to the Hospital Pharmacy (1st Floor, Inpatient Ward).
Bring the drug ticket to the drug counter (1st floor, Inpatient Ward).PDF

Out-of-Hospital Prescription (Standard Procedure)

Please take your prescription to any community pharmacy
(dispensing pharmacy or prescription-receiving pharmacy).
